Summary Care Record

Derbyshire County and Derby City GP Practices are taking part in the Summary Care Record programme.

What is the Summary Care Record

If you are registered with a GP Practice in either Derby City or Derbyshire County (and you will be 16 or over by June 2012) you will receive an information pack. Your pack will contain

  • A letter explaining the Summary Care Record and details on where to get further information
  • A leaflet that explains how the Summary Care Record will be used to support your emergency care, how we will control who can see the information held, where you can get more information about Summary Care Records and your choices, and how to access leaflets in other languages and formats

Choosing  to have a Summary Care Record

If you agree to have a Summary Care Record you do not need to do anything

Choosing not to have a Summary Care Record

If you choose not to have a Summary Care Record you will need to return the opt-out form you received in the post.  If you need to get copy of the opt-out form:

  • Contact your GP practice
