Older Peoples Services

Maximising the quality of life for Older People in Derbyshire

There are 137,000 people over 65 years old in Derbyshire. This number will gradually rise as the population ages. NHS Derbyshire County and Derbyshire County Council are working together to meet the health and social care needs of older people in Derbyshire.

Commissioning Older Peoples Services

Derbyshire County Primary Care Trust is committed to planning and buying (commissioning) services to meet the health needs of older people.

Some people's health needs will be met through specialist services and for other people more general services will be suitable. See below for further information about the services we commission. This web page is under development and further information will be added soon.

Dementia Services

Dementia is a condition of the brain which causes a gradual loss of mental ability. In the area of dementia, improvements to services in Derbyshire are in hand and a Dementia Care Pathway is being revised along the lines of current guidelines from NICE (National Institute for Health and Clinical Excellence) and the National Dementia Strategy - 'Living Well with Dementia'. Derbyshire has formed a joint dementia commissioning group which is represented by NHS Derbyshire County, Derbyshire County Council, Alzheimer's Society and General Practitioners. This group will be overseeing the development of new services for people with dementia and their carers.

Falls Prevention Services

Falls are not a consequence of old age; rather they are nearly always due to one or more underlying risk factors. Recognising and modifying these risk factors is crucial in preventing falls and injuries. All Health care providers e.g. your GP, District Nurse, Occupational Therapist, Physiotherapist and Pharmacist will be able to give you advice about fall prevention, some people with more complex needs my be referred to a Specialist Falls prevention service.

Intermediate Care

Intermediate care is a function rather than a discrete service. This means that the term 'Intermediate Care' incorporates a wide range of different services. Teams which deliver Intermediate Care will support anyone with a health-related need through periods of transition; this could be between hospital, community health services, primary care and long-term care and links to social care re-ablement.

Bone health and Osteoporosis

Osteoporosis causes fragile bones, which can lead to painful and disabling fractures. It is a long-term condition that is currently putting 2.3 million people in England at risk of painful fractures, which are costly for the NHS to treat. Much can be done to prevent these fractures, through the proper identification, treatment and care for people who have osteoporosis and or are at risk of falls.
