I am writing to find out the number of patients' medical records that have gone missing within your organisation.

Specifically I would like to know:
1) Every instance where a record/document has gone missing.
2) For each instance a brief description of what happened and what the resolution was (e.g. Laptop was lost on train, files were mislaid etc…)
3) For each instance how many records went missing. If an exact figure is not possible an estimation of how many disappeared is OK.

I am undertaking research relating to ambulance activity for diabetic patients. Please would you supply the following data relating to patients registered in your PCT for the time period 1st July 2010 – 31st June 2011?
Ideally I would like to know for patients with Type 2 Diabetes only:
1.      The total number of call outs due to a hypoglycaemic episode?
Of these call outs:
2.      The total number of hypoglycaemic patients treated at the scene?
3.      The total number of hypoglycaemic patients that were transported to hospital?
4.      The total number of hypoglycaemic patients that were transported to another centre?
However, if the data can’t be split by Type 1 and Type 2 diabetes I would be interested in the same questions being answered for diabetic patients irrespective of Type.
Finally, if the data can’t be split by Type and reason for call out i.e. hypoglycaemic episode I would be interested in the same questions being answered just for diabetes.

• Ethnicity.
• Experience of racism ( prejudice, discrimination, verbal abuse) and the possible harmful consequences its impact can cause.

a) What difficulties can arise for staff and service users when faced with discussions of the above, sometimes stigmatised, issues.
b) What impact does having to discuss the above, sometimes, stigmatised issues have on BAMEs:-
• Health and social wellbeing.
• Ability to access health and/or social services. ( Do you have information showing the above discussions can act as a barrier to individuals accessing services.)

What are the potential health and social consequences for BAME individuals as a result of them experiencing years of racial/ethnic isolation.

NB. Racial/ethnic isolation; that is BAME individuals:-
Brought up and/or living in areas and/or families that are overwhelmingly white.
